How to become a new patient

Thank you for your interest in our pediatric practice. We focus on general pediatric and adolescent care from growth and development milestones to treatment management of acute and chronic illnesses. For specialized conditions, we coordinate care with specialists to ensure your child receives the best possible care. We accept new patients from newborns to 18 years of age. Established patients may continue care until their 26th birthday.

Read our practice booklet and complete the registration forms linked here

Get a copy of your child’s medical records from your current provider

  • Bring them to our office OR
  • Mail them to us before your first appointment

We’ll contact you to set up your first appointment

Feel free to call our office with any questions or concerns. Our staff will gladly assist you through the registration process. If there are any concerns our staff cannot address, a telehealth visit can be scheduled with our nurse practitioner.

  • “Dr. Simon has been caring for my children for over 10 years. He is very knowledgeable and kind. He is great with both my kids, ages 15 and 10. We just left my daughter’s annual physical(age 10) and when we got in the car, she said ‘I love Dr. Simon!’ His staff has been with him for years and we love them, too!
